The approach we take to best meet the needs of our target population has three phases:
1. MEETING BASIC NEEDS
Upon entry to our residences Praxis Housing Initiatives provides clothing, food and emergency medical care for all residents who require them. Clients are then assigned clean, safe, rooms. Once clients are settled, we conduct thorough interviews to assess individual needs. Clients are seen by doctors who will be their primary care providers while they are at Praxis and later on as they reenter the community as part of a continuum of care. Mental health assessments are conducted through mental health partnerships.
2. ONGOING SUPPORT
Our aim is to prevent decline or relapse into homelessness by working with clients to build an unbroken chain of support that begins at Praxis, one that acts as a buffer between them and the streets.
3. INTENSIVE INTERVENTION
Clients and case managers put together individualized "action plans" that help guide the clients towards self-reliance, and leaving homeless in their past. Case workers and other social services staff are assigned to follow up with clients between weekly meetings to help them reach the benchmarks set in the action plans.
What We Do
Praxis offers transitional housing, health and social services to homeless men and women with AIDS and other special needs. Our residents are most vulnerable, with histories of physical and mental illness, Hepatitis C, TB, AIDS, addiction and histories of incarceration. Besides housing, we provide physical tests and vacinations, health referrals, case management, crisis intervention, vocational training, harm reduction skills, and independent living skills.
